Just fill in the application on the right with your organization’s information.  You will then be able to download the Partner Application Packet.

Blusource Partner Application Process:

1. Fill out the downloaded application and provide the following documentation.

  • A copy of your state and federal tax-exempt 501(c)(3) letters.
  • A copy of your current state sales tax permit.
  • Credit Card Authorization Form (credit card will be charged as payment for the first 5 orders of $100.00 or more in order to establish a $500 credit line with us, and to be kept on file in the case that an invoice goes over 30 days past due).
  • A signed Terms of Accepting Product Donations form.
  • A copy of your most recent 990-tax form or annual report.

2. For quicker processing you may fax your completed application and required documentation to:

3. Once we have received your application, it will take 2-3 business days to process.

4. Upon approval, your account manager will e-mail you a temporary user name and password. At this time you will be able to access the online catalog as well as update your account information.

5. You will receive bi-monthly e-mails regarding catalog updates and special offers. If you have any questions about products or donated items, your personal account manager will always be ready to help you.
